QUALITY ASSURANCE PROCESS

                                                  

                                                             

The Organization obtains input from the consumers, parentís/guardians and support staff in the following ways:

1. Through participation in regularly scheduled meetings with consumers, parents/guardians and support staff to discuss the consumerís plan of  service care.

 

2. Within 30 days of admission into the program, an individualized service Plan (ISP) shall be developed according to State requirement. Medical examination and other assessments will be completed within 6 months of admission in line with State requirements.

 

3. Satisfaction surveys or interviews are conducted annually, with all consumers and their families, staff, Advisory Board members, and referral agents. 

4. On an ongoing basis, information about the program are obtained directly from the consumers and by  telephone from their  parent/guardian.

5. Information are also gathered during social events arranged by the Organization, e.g.,  during the annual holiday party.
